This policy supplements our Privacy Policy. "Consumer health data" has the meaning given by applicable law and generally includes personal information linked or reasonably linkable to a consumer that identifies past, present, or future physical or mental health status.
1. Scope
This policy applies where state consumer health privacy law covers Visionne's processing. It does not replace a licensed provider's privacy notice or a HIPAA Notice of Privacy Practices when one applies to that provider.
2. Consumer health data collected
- Eyeglass or contact lens prescription values and prescription images.
- Pupillary distance, lens measurements, optical needs, and requested vision correction.
- Contact lens brand, parameters, and reorder information.
- Renewal eligibility responses, reported vision stability, symptoms, and relevant history you choose to provide.
- Information that links an order, provider interaction, or product request to you.
- Inferences necessary to determine product compatibility or service eligibility.
We ask you not to send health information through general contact or privacy request forms unless specifically requested through a secure workflow.
3. Sources
We collect consumer health data directly from you; from a prescriber or licensed provider when you direct or authorize the exchange; from a person authorized to act for you; and from product, order, or support systems used to complete your request.
4. Why we collect and use it
- Validate and fulfill prescription products and custom lens work.
- Assess frame and lens compatibility and manufacturing requirements.
- Route requested renewal information to a licensed provider and communicate the result.
- Provide support, remakes, returns, safety notices, and order records.
- Secure services, prevent fraud, comply with law, and establish or defend legal claims.
We do not use consumer health data for unrelated advertising or to infer characteristics unrelated to your requested vision service.
5. Consumer health data sharing
We may share the minimum reasonably necessary data with optical laboratories and product suppliers; licensed providers involved in a service you request; secure hosting and communications vendors; carriers when needed for fulfillment; professional advisers; and authorities or other parties when legally required or needed to protect rights and safety.
Specific recipients depend on your order and service. You may request a list of categories of third parties and, where applicable law requires, specific recipients through Your Privacy Choices.
6. Sale and authorization
Visionne does not sell consumer health data. We will not sell consumer health data without first obtaining a separate, valid authorization where required. We obtain consent when applicable law requires it for collection or sharing beyond what is necessary to provide a product or service you requested.
7. Your rights
Depending on applicable law, you may confirm whether we collect, share, or sell consumer health data; access the data; obtain information about sources and recipients; withdraw consent; request deletion; and appeal a refusal. Deletion may be subject to legal exceptions and may prevent us from completing a requested order or supporting a warranty.
Submit a request through Your Privacy Choices or call 646-358-2763. We will authenticate requests using commercially reasonable methods and will not unlawfully discriminate against you for exercising a right.
8. Retention, security, and changes
We retain consumer health data only as long as reasonably necessary for fulfillment, support, safety, legal compliance, and dispute resolution. We use safeguards designed for the sensitivity of this information, including private storage for submitted order and prescription files. No system can be guaranteed completely secure.
Material changes to this policy will be posted and, where required, consent will be obtained before applying a new practice. Contact: Visionne Work., 167 Madison Avenue, New York, NY 10016, nyc@visionnework.com.
